I am sure someone got a second one for physical contact with a player and/or official after the first, but considering thy actually restarted play it has got to be close.  What a numbskull

DiMaria did for MU against Arsenal.  He dove and ref gave him yellow and while ref was writting his number down, DiMaria grabbed him by the hand and turned him to get attention.  A huge no no in EPL, he got a 2nd yellow and was told to leave.

As long as it happened to MU it works for me
