Two, how exactly does a player tell the head coach "no" unless the head coach is a weakling?

By having the owner on the QB's side and not letting the head coach make the hard decisions. It's not being a weakling. Cam probably had Richardson backing him. It, unfortunately, happens too often in sports. The team's 'star' has too much influence with the owner. Just look to GB as a recent example of star power.
