It is hard to tell from this video. go back a couple more years and then compare to now. The decline is too large to be excused with age. There is a flare almost every throw.

Also surgeries take time to bug people and effect them. My sis has a metal rod in her leg didn't effect her for two years, now, after normal use she has to limit herself... and that is what is happening to Jake he has to limit himself and thus you see it clearly in the play calling.

Even back then he couldn't lead a receiver on a deep go route. Receiver had to turn completely around basically everytime to catch the ball. Only deep throw he seemed capable of leading the receiver on was a post route and that was a 50/50 proposition as well.
