The gifts that get the best reaction? A simple card with a heart felt letter hand written to her conveying how much she means to you, what the day means to you, and what the future will be.

I was going to suggest the photo and engraved frame idea, but others beat me to it. It doesn't even necessarily have to be the two of you together. It could be a photo, blown up/edited/whatever to make it look good, of one of her favorite moments from childhood.

For example, if she has a grandparent that she enjoyed spending time with as a kid, ask her parents for a good photo of the two of them together. Have it done up, etc. and like Zod said, include a hand-written note with it when you give it to her. I think the effort you have gone through with this is a gift she'd enjoy and treasure, and the photo something she could display for years.

Zod has the best idea. A heartfelt, card with a handwritten note telling her how much she means to you and how you can't envision spending your life without her in it. Maybe do something cheesy like handmade coupons redeemable for a backrub or a massage or something.
