The funny thing is the thumbs up/down thing is American not Italian. There are no historical records/sources that say anything of the sort and fights to the death were incredibly rare as Gladiators were expensive and no one was in the business of losing their expensive and well trained warriors for show.

Also the Italians say that thumbs down would mean put your sword down and thumbs up would mean raise your sword for the killing blow in the rare fight to the death if it occurred. We get the current Hollywood thing from a painting depicting a Gladiatorial fight.

Source: wife, kids, otherside of the family is Italian and most still live there.

But thumbs down anyway.
